NO SHOW & CANCELLATION POLICY
A fee of $50.00 will be assessed for all missed appointments and those not cancelled one business day prior. These charges will be your responsibility and billed directly to you. This will be due prior to your next appointment. Failure to keep multiple appointments may result in you being discharged from the practice.
