Advanced Directive Questionnaire

An Advance Healthcare Directive is a legal document in which a patient specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or incapacity. This is a legal document that is valid throughout the country.

Federal regulations require Athens Spine Procedural Center, LLC to have a patient’s Advance Directive on file if one has been completed or to provide official state advanced directive forms for your review if requested.

Athens Spine Procedural Center, LLC has established a policy to initiate resuscitative or other stabilizing measures and transfer the patient to St. Mary’s Healthcare System or Piedmont Athens Regional Medical Center for further evaluation in the event of an emergency.   The patient’s Advance Directive, if available, will be forwarded to St. Mary’s Healthcare System or Piedmont Athens Regional Medical Center where further treatment or withdrawal of treatment measures already begun will be ordered in accordance with the patient’s wishes, advance directive, or healthcare power of attorney.

After Hours Emergencies

Our calls are answered by an answering service after office hours for urgent medical issues. The answering service cannot process scheduling questions and/or cancellations or medication refills. These requests are handled during office hours ONLY (Phone Call Policy). If you have a true medical emergency dial 911 or go to the nearest emergency department.
